Tunstall Lifecare - TeleHealth

Telehealth is the remote monitoring of a patient’s vital signs and health condition from their own home. Ideal for people living with chronic health conditions such as CHF, COPD, CVD, Asthma and Diabetes, telehealth collects the patients’ vital sign and interview information for review on a regular/daily basis.

Telehealth offers a way of delivering tailored care for patients with long-term conditions in their own homes, helping improve quality of life and prevent avoidable hospital admissions.

The data collected is securely transmitted to the triage monitoring centre for review by a clinician. Abnormal readings for the patient are flagged for follow up and intervention such as clinical coaching, nursing visit or GP appointment. The collected data may also be passed on to the patient’s doctor for further review or intervention.

Telehealth allows the patients’ care team including community nurses, GPs and specialists to closely monitor the patient’s condition and intervene before it reaches crisis level and results in hospitalisation. Telehealth also offers video conferencing at the press of a button, allowing patients to maintain face to face contact with their clinician regardless of their location or medical state.

Telehealth may be used for, but is not limited to:

• Early hospital discharge
• Remote communities where access to doctors is limited
